Issuing National Identity Cards
- Relevant Application form obtaining a Identity Card (New Identity Card - DPR 1, Obtain a duplicate - DPR 7, Amendment of Identity Card - DPR 8)
- Original Birth Certificate or Probable Age Certificate and a photocopy for the applicants age below 50 years
- For age above 50, If there is no birth certificate or probable age certificate, results searching sheet and optional documents should be submitted (Leaving Certificate of school, Certificate of Baptism, Birth certificate of a child stating the date of birth of the applicant, Excerpt from the birth certificate, estate leave certificate, certified copies of the relevant pages of passport. , Registration card issued by the estate superintendent, birth certificate)
- Citizenship certificates if born abroad
- Color photos 6 (13/8 "* 7/8")
- If you are applying for an NIC for the first time, the photocopy of the National Identity Card of the applicant's mother/father/guardian certified by the Grama Niladhari
- Marriage certificate to obtain the name of the husband
- If the name of a Buddhist monk, Samanera Certificate or Upasampada Certificate required
- Certificate issued by the Department of Buddhist Affairs if the Buddhist monk is a subsistence
- If the duty post should be included in the identity card, a service certificate obtained within a period of 03 months is required
- The original copy of the complaint made to the police regarding a lost identity card
- The current Identity card is required to apply for an Amended Identity Card
- Stamps, 16-17 Rs. 3 / =, Over Rs. 13 / = Revisions and lost Rs. 15 / =
- Formal affidavit with stamp (if needed)
Inclusion/Alteration Of The Name Of a Birth Certificate
For those over 7 years -
- The Christian Certificate - Student Performance Index
- Newspaper advertisement - Bank Book
- National / Postal Identity Card - Marriage Certificate
- Detailed Report from the government or other institution
- Certified copy of voting register
You must submit at least three of the above documents
other than that
- Relevant Application.
- Original birth certificate to be amended.
- Grama Niladhari letter signed by the relevant Divisional Secretary stating that both names are used for the same person.
To be considered,
- If the name change is intended for a person under the age of 21, the statement must be submitted by the parent / guardian.
- The new name must have been in use for at least a year.
Children below the age of 7 years,
- Original birth certificate to be amended
- Parent's Marriage Certificate
- Mother's / Father's National Identity Card
- Affidavit stating that the mother and father have no objection to the relevant amendment
This declaration should be handed over by the child's mother/father/guardian.

Correction of a Mistake that Occurred in The Mother's coloum of a Birth Certificate
- Original birth certificate to be corrected
- Mother's original Birth Certificate
- Copy of mother's National Identity Card
- Original Marriage Certificate of the parents
To include/ rectify the father's coloum/ grand farther's coloum in a birth certificate
- The original copy of the Birth Certificate which can be corrected
- Father's original Birth Certificate
- Parent's Original Marriage Certificate
- Copy of Mother's/ father's National Identity Card
Registration of past Birth's
- Birth Description (Original)
- Photocopy of manual injection card
- Original marriage certificate if the parents are married
- Original Birth Certificates of Parents
- Photocopies of National Identity Card of Mother and Father
- If the birth is less than 1 year, stamps of Rs. 50
- Stamps of Rs. 100, if more than 1 year
Please bring the above documents to the District Registrar's Office with Parent and child
Registration of past death's
- In case of death in a hospital, the Original Death Certificate provided by Hospital.
- Certificate of Grama Niladhari
- Condolences notices/ death notices
- Receipt of the coffin
- Letter from the Chief Incumbent / Pastor who performed religious rites
- If the death is less than 1 year, stamps of Rs. 50 and more than 01 year, stamps of Rs. 100
- Copy of the Birth Certificate / Marriage Certificate and National Identity Card of the Publisher
Please come to the District Registrar's Office on Monday or Wednesday with the above documents.
